Epigastric pain remains the most misleading reason for consultation in gastroenterology. The stomach and pancreas share the same projection territory, the upper epigastric region, which regularly delays differential diagnosis. However, the behavior of the pain in relation to meals, its irradiation, and the associated biological markers allow for reliable differentiation right from the initial management.

Pain rhythm in relation to meals: the underestimated discriminating criterion

Gastric pain (ulcer, gastritis, functional dyspepsia) follows a typical postprandial or hunger rhythm. It appears in the minutes following ingestion, or conversely, after a distance from the meal (hunger pain from a duodenal ulcer), and often calms down with a gastric bandage or a proton pump inhibitor.

Pancreatic pain does not adhere to this pattern. It sets in continuously, worsens after a fatty meal, but never completely disappears between meals. We frequently observe that patients describe a pain that “doesn’t let go,” including at night, which immediately points towards the pancreas.

  • Involuntary weight loss over several weeks, without dietary changes
  • Episodes of chronic diarrhea resistant to usual symptomatic treatments
  • Onset of secondary diabetes due to progressive destruction of the islets of Langerhans

These signs of exocrine and endocrine insufficiency do not exist in pure gastric pathology. Their presence definitively points towards the pancreas.

First-line biology: amylase, lipase, and absence of gastric anomaly

The updated European recommendations on acute pancreatitis (United European Gastroenterology, 2023) highlight a decisive practical point. A true pancreatic pain is accompanied by a clear and rapid elevation of blood amylase and lipase. Lipase, more specific to pancreatic tissue, remains the reference marker.

In contrast, functional stomach pains or gastritis do not cause any pancreatic biological anomalies. This contrast allows the physician to make a quick decision in emergencies, often even before imaging.

Frequent diagnostic trap

We recommend not to rely solely on the location of the pain. An early pancreatitis can mimic a common gastritis for several days, with moderate, epigastric pain, without clear irradiation. Serum lipase measurement should be systematic in the presence of any persistent epigastric pain beyond 48 hours, especially in the presence of risk factors (alcohol consumption, biliary history, hypertriglyceridemia).

Imaging and distinction between stomach and pancreas: ultrasound and CT scan

Imaging confirms or refutes the clinical and biological hypothesis. Abdominal imaging studies published since 2022 show that the combination of epigastric pain, bar-like irradiation, and a hypoechoic pancreas on ultrasound or altered on CT scan constitutes a major practical criterion for distinguishing a pancreatic origin from a gastric origin.

The stomach often appears normal on imaging in cases of pancreatic pain, which reinforces the differential diagnosis. Abdominal CT with contrast remains the reference examination for acute pancreatitis: it assesses the extent of inflammation, possible necrosis, and local complications.

For gastric pathology, upper digestive endoscopy (gastroscopy) remains the first-line examination. It directly visualizes the gastric mucosa and allows for biopsy. The pancreas and stomach therefore do not share the same diagnostic pathway, which helps the clinician structure their assessment.

  • Isolated epigastric pain, rhythmically related to meals, without back irradiation: gastroscopy as a priority
  • Continuous pain with bar-like irradiation, worsened by fatty meals: lipase measurement followed by abdominal CT
  • Atypical pain persisting beyond 48 hours despite antacid treatment: systematic lipase measurement to rule out early pancreatitis

The diagnostic delay between gastric and pancreatic pathology remains a documented issue. Many pancreatitis cases, both acute and chronic, are initially labeled “gastritis” or “reflux” because the enzymatic measurement was not requested. Keeping these criteria in mind – food rhythm, irradiation, biology, imaging – allows for the correct hypothesis to be made from the first consultation and avoids a detrimental loss of time.
